Multiple Text Box Question Type

The Multiple Text Boxes question (named "Custom Form" in the builder) allows you to collect multiple open-ended answers with a single question. You can choose an unlimited amount of labels that a respondent will fill out. This question is best used when you want to gather any type of open-ended response for a group of similar categories.

For this question you can choose the following:

• Require an answer (can not proceed until answered)
• Require a specific format (like email or phone number)
• Validate based on a list, such as employee number

Responses will be displayed in a table like format where you can see the responses for each answer label along with a link to view the respondent answers.

The Excel export will have a column for each label and answer choice.
